How to Plan for Your Future

Our goal at the Wallace Center is to improve the quality of life for Greenwich residents who are 55 and older through the planning and coordination of innovative programming and services, including long-term care planning and advance directives.

Why Are Advance Directives Important?

Advance care planning can help you get the care you want for future medical treatment by putting your preferences in place or designating someone to make your decisions for you. Called advance directives, completing these legal documents not only ensures you have the ‘advance’ choice but it could also help your loved ones grieve more easily and feel less burden and/or guilt.

If you're an older adult living in Greenwich, or a caregiver in need of resources, there are steps you can take to help you get started, including:
  • Department of Health & Human Services’ Advance Care Planning, A Conversation Guide (free to download)
  • State of Connecticut’s Advance Directive Forms (free to download.) To complete an advance directive, download form link above and bring the completed form to a notary public or an attorney.
